Job Description

Home Health care RN Case Managers to assess, record and monitor patient's plan of care, skilled nursing, communicate with Physicians, educate and instruct patient and family. Complete all documentation in a timely manner.

Qualifications

Minimum 2 years hospital experience, current TB, CPR, fingerprint clearance card, driver's license, auto insurance.

Education

AZ RN State License/Valid Compact License
